  • Three\xc2\xa0letters of reference\xc2\xa0on the forms provided in the application packet. Preferably, the references will be from employers, instructors, or other persons who can assess the applicant\'s academic and professional potential.   • Personal statement. This is a brief essay outlining the applicant\'s reasons for wanting to pursue graduate education in health informatics, career objectives, and any other information that would help the admissions committee get to know the applicant.   • Satisfactory criminal\xc2\xa0background check:\xc2\xa0 After being accepted into the program, all incoming students are required to complete a background check. A one-time fee must be paid directly to the company performing the background investigation.\xc2\xa0 Instructions will be emailed to students when they are accepted to the program.
