- A copy of any previously issued U.S. visas, I-94 forms, and I-20 or DS-2019 forms (if applicable)
- Proof of English proficiency. Official English proficiency examination results must be forwarded directly to IPFW by the testing agency. The most common way of demonstrating proficiency in English is by taking one of the following tests:
