  • copy of a visa or residence permit, if available at the time of application.

  • official translation of the diploma into Polish
  • original diploma supplement / transcript of records and its official translation into Polish (unless it was issued in English)
  • certificate confirming your proficient command of English (list of certificates)
  • recent health certificate issued by a medical doctor, stating that there are no contraindications for your undertaking studies (if you use another health certificate form you must submit its official translation into Polish)
  • motivation letter (600-700 words) (only for International Business).
